对比观察保留乳头乳晕的乳腺癌改良根治术、传统乳腺癌改良根治术的临床应用价值

摘要：目的：分析在乳腺癌患者的治疗中应用保留乳头乳晕乳腺癌改良根治术或者传统乳腺癌改良根治术的治疗价值。方法：对照组行传统乳腺癌改良根治术治疗，同期观察组采用保留乳头乳晕乳腺癌改良根治术治疗。结果：乳房美容学优良率观察组为91.89%，对照组为70.27%，P<0.05;术后并发症率观察组为8.11%，对照组为21.62%，P<0.05;术后随访中局部复发率和远处转移率2组对比P>0.05;术后3个月末经评估FACT-B量表中家庭维度、情感维度、生理维度、功能维度以及总得分观察组均较对照组更高P<0.05。结论：对于乳腺癌患者在治疗中采用保留乳头乳晕乳腺癌改良根治术可获得确切的治疗效果，有助于改善患者术后的生活质量、降低并发症率，并获得满意的乳房美容学效果。

Abstract： Objective： To analyze the therapeutic value of modified radical mastectomy with nipple and areola preservation or traditional modified radical mastectomy in the treatment of breast cancer patients. Methods： The control group was treated with traditional modified radical mastectomy， while the observation group was treated with modified radical mastectomy with nipple and areola preservation. Results： The rate of excellent and good breast cosmatology was 91.89% in observation group and 70.27% in control group， P < 0.05. The postoperative complication rate was 8.11% in the observation group and 21.62% in the control group （P < 0.05）. The local recurrence rate and distant metastasis rate in postoperative follow-up were compared between the two groups （P > 0.05）. At the end of 3 months after operation， the family dimension， emotional dimension， physiological dimension， functional dimension and total score of fact-B scale were higher in the observation group than in the control group （P < 0.05）. Conclusion： For breast cancer patients， modified radical mastectomy with nipple and areola retention can achieve definite therapeutic effect， improve postoperative quality of life， reduce complication rate， and obtain satisfactory cosmetic effect of breast.

乳腺癌是在女性群体中非常常见的乳腺恶性肿瘤疾病，近年来随着女性的工作压力以及生活压力不断增加也使得该疾病的发病率有所上升。乳腺癌根治术是对于乳腺癌患者的重要治疗手段，近年来临床诊疗技术迅速发展，这也使得乳腺癌患者能够在病变早期获得及时的诊断与治疗，对于改善患者的预后意义重大。传统乳腺癌根治术在应用中创伤性大，与此同时术后患者乳房缺失使得外在形象受到影响，伴随上肢活动障碍也对患者术后的生存品质造成了极大的干扰，所以需要积极的探寻效果确切以及创伤性较小的乳腺癌手术方案。……
