我院静脉药物调配中心不合理处方分析

摘要：目的探讨静脉药物调配中心(PIVAS)不合理处方的原因，提高临床合理用药水平。方法对我院2012年8月～2014年5月处方进行统计分析和归纳。结果审核102 092 份医嘱中不合理处方占411份(0.40%)，其中，溶媒选择不合理、溶媒用量不当、超说明书用药、配伍禁忌、给药途径不合理、医嘱录入错误、未达到治疗剂量等占主要因素。结论药师对不合理处方进行干预，减少不合理用药现象，保障患者用药安全，促进临床合理用药。

Abstract：ObjectiveTo analyze reasons of the irrational prescriptions that have been used in pharmacy intravenous admixture services (PIVAS)so as to promote rational drug use in our hospital.MethodsThe prescriptions of PIVAS in our hospital from August 2012 to May 2014 were analyzed statixtically in the article.ResultsAmong 102 092 prescriptions,411 prescriptions were irrational drug use,accounted for 0.40%,including improper solvent,unsuitable concentration,overdose,unsuitable combination,inappropriate route of administration,incorrect input of information and lower the effective dose.ConclusionPIVAS pharmacists can improve the use of drug by correcting irrational prescriptions and reduce irrational drug use effctively,which can ensure the safe medication of patients and improve the rational drug use.

我院静脉药物配置中心于2012年6月开始运行，在依据药物特性设计的操作环境下，由受过培训的药学专业技术人员，严格按照操作程序，进行包括全静脉营养液、细胞毒药物和抗生素等静脉用药物的配置，为临床药物治疗与合理用药服务。本文通过对我院2012年8月～2014年5月静脉药物调配中心的处方进行回顾性分析，进一步探讨我院PIVAS不合理处方的原因，为以后我院药师参与处方审核，及时发现并纠正不合理处方，减少药物不良反应的发生，为临床用药安全提供参考。

1资料和方法

1.1资料来源2012年8月～2014年5月，共收到长期医嘱102 092 份。

1.2方法依据处方管理办法、药品管理法、《新编药物学》第17版、《静脉药物配置中心临床服务与疑难精解》、《临床静脉用药调配与使用指南》，药品说明书及其他相关文献，对411份不合理处方进行归纳、统计、分析。……
