复方甘草合剂合用所致双硫仑样反应文献概述

复方甘草合剂；双硫仑样反应

复方甘草合剂，又名复方甘草口服溶液，具有镇咳、祛痰作用，主要用于治疗一般性咳嗽及“上感”所致咳嗽。近年来，国内有文献报道，复方甘草合剂与其他药物合用可致双硫仑样反应，现概述如下：

1 与头孢哌酮

李宏坡报道，患女，67岁，因社区获得性肺炎给予头孢哌酮钠2g静滴，2/日，复方甘草合剂20ml口服，3/日，连用11天后患者出现面部发红，手部发抖，查体：血压130/60mmHg,心率100次/分，心电图示窦性心动过速，即予对症治疗，2小时后上述症状逐渐缓解，生命体征平稳[1]。

2 与头孢哌酮舒巴坦

王菲菲等报道，患男，67岁，因胆囊结石行胆囊切除术后给予头胞哌酮舒巴坦3.0g静滴，2/日，因咳嗽给予复方甘草口服液10ml口服，3/日，用药约15分钟后，患者出现面部潮红，感胸闷，心悸，颜面及颈、胸部皮肤潮红，心电图示窦性心动过速，即予对症治疗，30分钟后患者症状逐渐缓解[2]。

3 与拉氧头孢

徐利等报道，患男，89岁，因慢性支气管炎急性发作给予注射用拉氧头孢1.0g静滴，2/日，为缓解患者粘痰，不易咳出，即予复方甘草口服液口服，用药5分钟后患者感胸闷、气短、颜面潮红、多汗，即停复方甘草口服液，予对症治疗后，上述症状逐渐缓解[3]。

4 与硝呋太尔

张敏等报道[4]，例1，患女，42岁，因霉菌性阴道炎给予硝呋太尔口服，已用5天，因“上感”咳嗽自购服用复方甘草合剂10ml，约20分钟后，患者突感心悸、胸闷、头昏恶心、呼吸困难，查体：面部皮肤潮红，咽部充血，心电图示窦性心动过速。即予对症治疗，约40分钟后，症状明显好转。例2，患男，37岁，因咳嗽咳痰自服复方甘草合剂3天，因泌尿系统感染给予硝呋太尔口服，服药后约3分钟，患者出现面部潮红，全身发热，胸闷伴恶心、呕吐，予对症治疗，约20分钟后上述症状逐渐消失。

复方甘草合剂中含有乙醇，与头孢类药物硝基咪唑类药物合用后，大量乙醛蓄积体内，乙醛作用增强，从而引起双硫仑样反应[3，4]。因此，因引起广大医护人员的注意，在使用本品前应详细询问患者饮酒史、过敏史、用药史，尤其是需要合用其他药物时，强调本品与头孢类药物、硝基咪唑类药物之间的相互作用，重视对患者的健康教育，告知患者使用复方甘草合剂应避免与头孢类药物、硝基咪唑类药物配伍使用，以防发生双硫仑样反应，保证用药安全。
