畲医发痧疗法在腰椎术后快速康复中的应用

[摘要] 目的 观察畲医发痧疗法在腰椎间盘突出症微创椎间孔镜术后快速康复中的应用疗效，并探讨其作用机制。方法 收集临床手术病例，采用随机数字表法将符合纳入标准并接受椎间孔镜微创手术治疗的60例腰椎间盘突出症患者随机分为治疗组及对照组各30例，对照组常规给予七叶皂甙钠针消肿，在疼痛评分大于3分时，临时给予帕瑞昔布针镇痛等对症处理。治疗组在此基础上采用畲医发痧疗法。比较两组效果优良率，记录术后主动直腿抬高时间、术后镇痛药使用次数。 结果 腰椎间盘突出症微创椎间孔镜术后，结合畲医治疗组术后主动直腿抬高时间明显提前，术后使用强效镇痛药物次数明显减少，经统计学比较分析有明显统计学意义（P<0.05）。两组治疗优良率比较有统计学意义（P<0.05）。 结论 畲医发痧疗法应用可促进腰椎间盘突出症椎间孔镜术后患者快速康复，减少镇痛药物的使用。

[Abstract] Objective To observe the therapeutic effect of She medicine headstroke therapy on postoperative rapid recovery after minimally invasive intervertebral foraminal mirror of lumbar disc herniation and to explore its mechanism. Methods Clinical surgical data were collected， and a total of 60 patients with lumbar disc herniation who conformed to the inclusion criteria and received minimally invasive intervertebral foraminal mirror were randomly assigned to the treatment group and the control group according to the random number table method， with 30 patients in each group. The control group was given sodium aescinate needle for detumescence. When the pain score was greater than 3， parecoxib was temporarily administered for analgesia and other symptomatic treatment was given. The treatment group was given She medicine headstroke therapy on the basis of the control group. The excellent and good rate of the curative effects were compared between the two groups， and the time of postoperative active straight leg raising and the application frequency of postoperative analgesics were recorded. Results After minimally invasive intervertebral foraminal mirror of lumbar disc herniation， treatment group combined with the She medicine， the time of postoperative active leg raising was significantly in advance， and the application frequency of potent analgesic drugs was significantly reduced. After statistical comparison and analysis， the differences were statistically significant（P<0.05）. The excellent and good rate compared between the two groups was statistically significant（P<0.05）. Conclusion The application of She medicine headstroke therapy can promote the rapid recovery of patients with lumbar disc herniation after intervertebral foraminal mirror and reduce the application of analgesic drugs.

腰椎間盘突出症（lumbar disc herniation，LDH）是临床常见病、多发病。据报道，本病的发病率为20%～30%[1]，且因其病程长、反复发作使患者工作和生活质量受到严重影响。目前椎间孔镜下髓核摘除术方兴未艾，该手术创伤小，直视下摘除突出髓核等致压组织，直接减压神经，同时未破坏脊柱稳定结构[2]，患者满意度高，但临床发现部分患者术后仍残留不同程度腰痛，本文在既往研究基础上，采用畲医干预部分椎间孔镜术后患者，取得更快的临床康复，现报道如下。

1 资料与方法
