锁定钢板内固定治疗跟骨骨折临床疗效评价

[摘要] 目的 探讨跟骨骨折患者行锁定钢板内固定术治疗的效果，评定其临床应用价值。方法 方便选取该院骨科室于2011年1月—2015年1月期间所收治的跟骨骨折患者120例，采用计算机表法的形式将其平均分成研究组和参照组，每组患者各为60例，其中研究组患者行锁定钢板内固定术治疗，参照组患者行常规经皮撬拨复位内固定术治疗，对比两组患者的手術用时时间，评价患者术后疼痛VAS评分指标、术后根骨复位情况、愈合时间以及功能恢复情况。结果 经过不同方法治疗后，研究组患者的手术用时时间（46.9±3.3）min短于参照组（90.7±5.2）min，其术后疼痛VAS评分（2.8±0.6）分、解剖复位优良率（98.3%）、骨折愈合时间（63.2±4.1）d以及功能恢复情况（96.7%）均优于参照组患者[（2.0±0.2）分、88.3%、（94.5±3.7）d、85.0%]。数据差异有统计学意义（P<0.05）。 结论 将锁定钢板内固定术应用于跟骨骨折病症治疗中，效果显著，且术后创伤小，且能够依据根骨部分的解剖特征进行塑性，这对于临床研究具有重要作用。

[Abstract] Objective To study the effect of locking plate internal fixation in treatment of intra-articular calcaneal fractures and evaluate its clinical application value. Methods 120 cases of patients with intra-articular calcaneal fractures admitted and treated in our hospital from January 2011 to January 2015 were convenient selected and randomly divided into two groups with 60 cases in each， the research group were treated with locking plate internal fixation， while the control group adopted the routine percutaneous prying reset internal fixation， and the operation time was compared between the two groups， and the postoperative pains VAS score index， postoperative intra-articular calcaneal reset， healing time and functional recovery of patients were evaluated. Results After treatment， the operation time in the research group was shorter than that in the control group[（46.9±3.3）min vs（90.7±5.2）min]， and the postoperative VAS score， anatomy and reset excellent and good rate， fracture healing time and function recovery situation were better than those in the control group， [（2.8±0.6）points， 98.3%， （63.2±4.1）d， 96.7% vs （2.0±0.2）points， 88.3%， （94.5±3.7）d， 85.0%]， and the differences between groups were statistically significant（P<0.05）. Conclusion The application effect of locking plate internal fixation in the treatment of intra-articular calcaneal fractures is obvious with small postoperative wound， which can form the plastic according to the anatomical characteristics of the root bone， and it is of important significance to the clinical research.

由于人体跟骨位置的形态极为特殊，且跟骨构造极为繁琐，因此一旦受到外力的重创，就会使跟骨本身的内部构造出现变形，出现压缩性骨折，因此跟骨骨折病症的发生不仅仅会造成跟距关节面位置的陷落，还会导致距下合并其他骨折病症的出现，且足部皮肤坏死的情况也常有发生[1]。在常规临床对跟骨骨折病症患者进行治疗的过程中，常应用经皮撬拨复位内固定术对患者进行救治，然而术后所产生的多种并发症，严重降低了患者的生活质量，因此该文将主要以锁定钢板内固定术为基础，探究将其应用于2011年1月—2015年1月期间所收治的120例跟骨骨折病症中的效果，现报道如下。……
