胫骨横向骨搬运结合负压引流治疗糖尿病足的临床疗效分析

【摘要】 目的 分析胫骨横向骨搬运结合负压引流（VSD）治疗糖尿病足的临床疗效。方法 20例糖尿病足患者， 均给予胫骨横向骨搬运结合VSD治疗， 对比患者治疗前后踝肱指数（ABI）及密歇根神经病变筛查量表（MNSI）评分以及患足功能情况。结果 治療后， 患者ABI高于治疗前， 足溃疡、足外观、踝反射、踇趾震动觉、尼龙单丝触觉评分均低于治疗前， 差异具有统计学意义（P<0.05）。治疗后， 患者正常运动功能评分（92.22±2.36）分、生活质量评分（93.13±2.14）分均高于治疗前的（89.31±3.14）、（88.85±3.16）分， 差异具有统计学意义（P<0.05）。结论 胫骨横向骨搬运结合VSD治疗糖尿病足有着显著的临床疗效， 可有效促进患者患肢周围血管及周围神经恢复、加速创面愈合， 具有较高的临床使用价值， 值得广泛的应用与推广。

【Abstract】 Objective   To analyze the clinical efficacy of tibial transverse bone transport combined with Vacuum sealing drainage （VSD） on diabetic foot. Methods   A total of 20 patients with diabetic foot all treated by tibial transverse bone transport combined with VSD. The ankle brachial index （ABI） and Michigan neuropathy screening instrument （MNSI） score and the function of the affected foot were compared before and after treatment. Results   After treatment， ABI of patients was higher than that before treatment， and foot ulcers， foot appearance， ankle reflexes， toe vibrations， nylon monofilament tactile score were lower than those before treatment， and the difference was statistically significant （P<0.05）. After treatment， the normal motor function score （92.22±2.36） points and quality of life score （93.13±2.14） points of patients were higher than those before treatment （89.31±3.14） and （88.85±3.16） points， and the difference was statistically significant （P<0.05）. Conclusion   Tibial transverse bone transport combined with VSD showed remarkable clinical efficacy in the treatment of diabetic foot， which can effectively promote the recovery of blood vessels and nerves around the affected limb and accelerate the wound healing. It has high clinical value and is worthy of wide application and promotion.

研究数据表明， 糖尿病足患者多并发糖尿病足溃疡， 有着较高的致残、致死率， 严重威胁患者的生命安全与身心健康。目前， 临床主要采取改善局部血运循坏、加速创面愈合的方式对患者予以治疗， 但治疗效率较低， 未能达到预期降低截肢率的目的。随着医疗水平的发展， 胫骨横向骨搬运结合VSD技术被应用到了糖尿病足的治疗中， 不仅手术简便、疗效显著， 且保肢成功率高。基于此， 本文对比和分析了胫骨横向骨搬运结合VSD治疗糖尿病足的临床疗效， 现报告如下。

1 资料与方法

1. 1 一般资料 选择2019年1月～2020年1月本院收治的20例糖尿病足患者作为研究对象， 其中男16例， 女4例;平均年龄（65.25±2.74）岁;平均病程（14.68±6.14）年;左糖尿病足13例， 右糖尿病足7例;患者均于入院后进行wagner分级[4]， 对溃疡深度和病因进行分类：2级4例， 3级9例， 4级7例。纳入标准：①符合糖尿病足临床诊断标准;②患肢创面未累及踝部;③所有患者均知情同意。排除标准：①存在心脑血管疾病、近期出现糖尿病酮症酸中毒等严重糖尿病;②患肢部位皮肤破损;③心肝肾功能障碍患者;④精神疾病、依从性差的患者。……
