疾病诊断相关分组(DRGs)病例组合指数(CMI)在口腔科绩效考核评价中的应用

摘要目的：探讨疾病诊断相关分组（简称DRGs）病例组合指数对医院口腔科绩效考核评价的影价值。方法：应用DRGs工具，比较医院2019年全年与2020年全年医院口腔科出院例数、总权重数与病例组合指数CMI的数值。结果：结果显示，2020年全年口腔科出院例数较2019年全年增加，总权重数降低，CMI值升高。结论：DRGs病例组合指数应用于口腔科绩效考核评价中可取得满意的效果，实现科室资源的合理应用，提高科室诊疗服务水平。

The Application of the Case Portfolio Index（CMI）for Diagnosis-related Ggroups（DRGs）in the Evaluation of Performance in the Department of Stomatology

AbstractObjective： To explore the shadowing value of the case portfolio index of disease diagnosis-related groups（DRGs）on the performance evaluation of hospital stomatology department. Methods： DRGS tool was used to compare the number of patients discharged from the stomatology department of hospitals in 2019 and 2020， the total weight number and the value of case combination index CMI. Results： The results showed that the number of patients discharged from the department of stomatology in 2020 was higher than that in 2019， the total weight number was lower， and the CMI value was higher. Conclusions： The application of DRGS case portfolio index in the performance evaluation of the department of stomatology can achieve satisfactory results，realize the rational application of department resources， and improve the level of diagnosis and treatment service.

疾病診断相关分组（Diagnosis Related Groups，DRGs）属于病例组合方式中的一种类型，主要是借助统计控制的基本理论进行患者的分类、分组同时实现统一管控。[1]DRGs最早的应用是在1983年的美国新泽西州，在实际的应用管理中，发现DRGs系统具有控制医疗费用支出、提高医院工作效率等多个方面的应用优势，在此后一些发达国家比如加拿大、澳大利亚、丹麦等也尝试应用，并且结合国情及医院实际进行一些改良及优化。[2]我国也较早的时间引进DRGs改变，试点城市选择首都北京，2008年设立北京版DRGs，收获满意的效果，后续关于DRGs用于绩效考核评价中受广泛关注，为提高医护人员工作积极性及优化医疗服务质量奠定坚实基础。[3]本院是一所三甲医院，口腔科人员数量众多并且分层合理，具备完善的管理系统，而为了适应医院发展的需求及满足人们日益升高的医疗服务需求，大力开展以DRGs为基础的科室绩效考核评价，依托医院信息技术提高管理质量。……
