对比分析B超引导下经皮肾镜双通道、腹腔镜下肾囊肿去顶术治疗单纯性肾囊肿效果

【摘 要】目的：分析在单纯性肾囊肿患者治疗中采用B超引导的经皮肾镜双通道手术或者腹腔镜下肾囊肿去顶术的治疗价值。方法：抽取2019年3月至2021年1月本院72例单纯性肾囊肿患者，随机数字表法进行分组，观察组采用B超引导下经皮肾镜双通道手术治疗，同期对照组采用腹腔镜下肾囊肿去顶术治疗，对比两组的常规围手术期指标、手术后的并发症发生率以及随访中的复发率。结果：观察组手术时间、失血量、术后拔管时间、VAS疼痛度评分以及住院时间等均较对照组更少（短）（P<0.05）；观察组术后并发症发生率为2.78%，对照组为13.89%（P<0.05）；观察组术后随访中肾囊肿复发率为2.78%，对照组为5.56%（P>0.05）。结论：对于单纯性肾囊肿患者在治疗中采用B超引导的经皮肾镜双通道手术可取得满意的临床疗效，有利于降低手术创伤，加快术后康复，并预防术后并发症。

Comparative analysis of the effect of dual-channel percutaneous nephroscope and laparoscopic renal cyst removal under the guidance of B-ultrasound in the treatment of simple renal cysts

【Abstract】Objective：To analyze the therapeutic value of B-ultrasound guided percutaneous nephroscope dual-channel surgery or laparoscopic renal cyst removal in the treatment of patients with simple renal cysts. Methods：A total of 72 patients with simple renal cysts in our hospital from March 2019 to January 2021 were selected and grouped by random number table method. The observation group was treated with B-ultrasound guided percutaneous nephroscope dual-channel surgery， and the control group was treated with laparoscopy during the same period. In the treatment of lower renal cysts， the conventional perioperative indicators， postoperative complication rate and recurrence rate during follow-up were compared between the two groups.Results：The operation time， blood loss， postoperative extubation time， VAS pain score， and hospital stay in the observation group were less （shorter） than the control group（P<0.05）； the postoperative complication rate was 2.78 in the observation group， and the control group was 13.89%（P<0.05）； the recurrence rate of renal cysts in the observation group was 2.78% in the postoperative follow-up， and 5.56% in the control group（P>0.05）.Conclusion：For patients with simple renal cysts， the use of B-ultrasound guided percutaneous nephroscope dual-channel surgery can achieve satisfactory clinical results， which is beneficial to reduce surgical trauma， speed up postoperative recovery， and prevent postoperative complications.

單纯性肾囊肿是近年来的临床常见病，病灶部位集中于患者肾皮质表面，其外层为纤维组织，内部囊液较为清亮，患者的主要症状为患区疼痛、身体发热以及泌尿系感染等。近年来，微创外科技术迅速发展，在单纯性肾囊肿患者的治疗中微创技术应用广泛，例如腹腔镜下肾囊肿去顶术的运用能够实现快速的肾囊肿减压以及手术的成功率较高。近年来经皮肾镜在临床泌尿外科应用广泛，有报道指出[1]经皮肾镜在单纯性肾囊肿患者的治疗中可获得确切的疗效。……
