从脾胃“失能”论治晚期前列腺癌

〔摘要〕 根据中焦脾胃“化”为主要功能的作用特点，得出恶性肿瘤是中焦失能的结果，而前列腺癌的发生与脾胃失能的关系更甚。桂枝汤调和阴阳，具象中焦，以桂枝汤为基础的方药调理中焦恢复脾胃“化”的功能，能益肾水而消癥瘕，对于晚期前列腺癌具有较好的治疗效果。

Theoretical Discussion on Treatment of Advanced Prostate Cancer from "Disability" of

Spleen and Stomach

〔Abstract〕 According to the function characteristics of the middle Jiao spleen and stomach “transformation” as the main function， it is concluded that the malignancy is the result of the spleen and stomach dysfunction of the middle Jiao， and the relationship between the occurrence of prostate cancer and the spleen and stomach dysfunction is more serious. Guizhi Decoction reconciles Yin and Yang， concretes middle Jiao， and Guizhi Decoction as based prescription recuperates middle Jiao to restore the “transformation” function of spleen and stomach. It can nourish the kidney and eliminate the cancer， and has a good therapeutic effect on advanced prostate cancer.

前列腺癌严重威胁男性健康，发病率居全球男性恶性肿瘤的第2位[1]。在我国前列腺癌患者确诊时，多到晚期，对于晚期前列腺癌的治疗，临床上使用内分泌治疗能取得较好的效果，但最终会进展为去势抵抗性阶段，使治疗陷于困境[2]。根据肿瘤的特性，笔者认为肿瘤产生的根本原因是是脾胃虚弱，而晚期前列腺癌也属于肿瘤的范畴，因此，晚期前列腺癌在治疗时应注重脾胃的健运，兹阐述如下。

1 肿瘤与中焦脾胃失能

中医学认为，脾胃五行属土，滋养人体全身。《素问·玉机真藏论》曰：“脾为孤藏，中央土以灌四傍。”脾胃的生理作用为以运化作用为基础，生水谷精微，化水谷之气，从而奉养全身，人身赖此而活，故为人体后天之本。人体之中，父精母血所化先天元气与脾胃所生水谷之气对生命具有决定性作用。且先天、后天之气浑然而成一体，通过气化活动调控人体所有的生命活动。《医法圆通·卷四·分脾胃为先后二天解》云：“人之运动，全在先天一团真气鼓动耳。……
