千金苇茎汤联合滋阴补肺汤对支气管扩张伴咯血的临床疗效观察及不良反应研究

【摘要】目的：探討千金苇茎汤联合滋阴补肺汤对支气管扩张伴咯血的临床疗效观察及不良反应研究。方法：选取2019年10月至2020年10月期间我院收治的78例支气管扩张伴咯血患者，随机法将78例患者分为2组，常规组和联合组，均39例，常规组口服滋阴补肺汤，联合组在上述治疗方案口服千金苇茎汤，连续口服14 d。观察患者疗效、外周血指标剂不良反应。结果：常规组患者总有效例数32例（82.05%），联合组总有效例数36例（92.31%），组间比较差异显著（c2=4.709，P=0.030）；未进行治疗时，常规组和联合组组患者外周血指标CRP、ESR和WBC相似（P>0.05），干预14 d后，两组患者CRP、ESR和WBC均降低，且联合组效果更优，组间比较差异显著（P<0.05）；治疗期间，联合组患者发生头痛、胸闷、胸痛、反胃恶心等发生率为15.38%，A组为20.51%，组间比较差异显著（P>0.05）。结论：千金苇茎汤联合滋阴补肺汤能够增加支气管扩张伴咯血患者临床总治疗效果，在一定程度降低患者外周血中CRP、ESR及WBC表达而减少炎症反应，且安全性较好，值得临床使用。

Qianjin Weigan decoction combined with Ziyin Bufei Decoction in the treatment of bronchiectasis with hemoptysis

[Abstract] Objective： To investigate the clinical efficacy and adverse reactions of Qianjin Weigan decoction combined with Ziyin Bufei Decoction in the treatment of bronchiectasis with hemoptysis. Methods： 78 patients with bronchiectasis and hemoptysis in our hospital from October 2019 to October 2020 were randomly divided into two groups， the conventional group and the combined group， 39 cases in each group. The conventional group was treated with Ziyin Bufei decoction， while the combined group was treated with Qianjin Weijing Decoction for 14 days. The curative effect， peripheral blood indexes and adverse reactions were observed. Results： there were 32 cases （82.05%） in the conventional group and 36 cases （92.31%） in the combined group， with significant difference between the two groups （c2=4.709， P=0.030）. Without treatment， CRP， ESR and WBC were similar in the conventional group and the combined group （P > 0.05）. After 14 days of intervention， CRP， ESR and WBC in the two groups were decreased， and the effect of the combined group was better than that of the control group The difference was significant （P < 0.05）； during the treatment， the incidence of headache， chest tightness， chest pain， nausea， etc. in the combined group was 15.38%， and that in the group A was 20.51%， with significant difference between the two groups （P > 0.05）. Conclusion： Qianjin Weigan decoction combined with Ziyin Bufei decoction can increase the clinical total therapeutic effect of patients with bronchiectasis and hemoptysis， reduce the expression of CRP， ESR and WBC in peripheral blood of patients to a certain extent， and reduce the inflammatory reaction， with good safety， which is worthy of clinical use.

支气管扩张症（Bronchiectasis）是慢性气道损伤引起支气管管壁肌肉和弹力支撑组织破坏所导致的一支或多支支气管不可逆性扩张。本病多见于儿童和青年，主要临床表现为慢性咳嗽、咳大量脓痰和反复咯血。本病过去发病率较高，仅次于肺结核，自抗菌药物和疫苗问世以来，该病的发病率已有明显下降。支气管扩张症并非一种独立的疾病，临床上，多种直接或间接影响支气管壁防御功能的疾病均可导致支气管扩张症。……
