【摘要】 目的:分析包頭地区2型糖尿病(T2DM)合并内脏型肥胖(VO)的影响因素。方法:收集2018年12月-2020年6月本院内分泌科入组国家标准化代谢性疾病管理中心(MMC)的T2DM患者393例,采用生物电阻抗方法测量内脏脂肪面积(VFA)和皮下脂肪面积(SFA),根据VFA值的高低将T2DM患者分为VO组(VFA≥100 cm2)246例,非VO组(VFA<100 cm2)147例。记录患者的一般临床资料以及T2DM并发症,并分析T2DM并发症的相关因素。结果:VO组BMI、WHR、VFA、SFA、餐后2 h C肽、胰岛素抵抗指数(HOMA-IR)、TG、血尿酸(UA)均明显高于非VO组,差异均有统计学意义(P<0.05);VFA、SFA均与体重、BMI、WC、HC、WHR、DBP、FCP、餐后2 h C肽、HOMA-IR、TG、UA呈正相关(P<0.05),VFA与身高、Cr均呈正相关(P<0.05),SFA与SBP呈正相关(P<0.05)。多元线性分析显示年龄、身高、体重、BMI、WC、HOMA-IR、TG与VFA增大密切相关,是T2DM合并VO的影响因素(P<0.05)。二元logistic回归结果显示,男性、身高<170 cm、腰围≥95 cm、BMI>30 kg/m2、并发高血压、高脂血症、脂肪肝是T2DM合并VO的独立危险因素(P<0.05)。结论:T2DM合并VO发生风险与性别有关,男性较女性发生风险高;血脂异常、胰岛素抵抗与VFA、SFA有明显的相关性;随着VFA的增大,T2DM患者并发高血压、脑卒中的风险也增大。

[Abstract] Objective: To analyze the influencing factors of type 2 diabetes mellitus (T2DM) combined with visceral obesity (VO) in Baotou area. Method: A total of 393 T2DM patients enrolled in the national standardized metabolic disease management center (MMC) from December 2018 to June 2020 were collected. The visceral fat area (VFA) and subcutaneous fat area (SFA) were measured by bioelectrical impedance method. According to the VFA value, T2DM patients were divided into VO group (VFA≥100 cm2) 246 cases and non VO group (VFA<100 cm2) 147 cases. The general clinical data and T2DM complications were recorded, and the related factors of T2DM complications were analyzed. Result: BMI, WHR, VFA, SFA, 2 h postprandial C-peptide, HOMA-IR, TG and UA in VO group were significantly higher than those in non VO group, the differences were statistically significant (P<0.05); VFA and SFA were positively correlated with body weight, BMI, WC, HC, WHR, DBP, FCP, 2 h postprandial C-peptide, HOMA-IR, TG, UA (P<0.05), VFA was positively correlated with height, Cr (P<0.05), while SFA was positively correlated with SBP (P<0.05). Multivariate linear analysis showed that age, height, weight, BMI, WC, HOMA-IR and TG were closely related to the increase of VFA and were the influencing factors of T2DM complicated with VO (P<0.05). The results of binary logistic regression showed that male, height<170 cm, waist circumference ≥95 cm, BMI>30 kg/m2, hypertension, hyperlipidemia and fatty liver disease were the independent risk factors of T2DM complicated with VO (P<0.05). Conclusion: The risk of T2DM complicated with VO is related to gender, and the risk is higher in men than in women; dyslipidemia and insulin resistance are significantly correlated with VFA and SFA; with the increase of VFA, the risk of hypertension and stroke in T2DM patients also increases.

2型糖尿病(type 2 diabetes,T2DM)及其并发症对患者生活质量和生存时间的影响越来越大。近30年,全球超重和肥胖的患病率不断上升,成人约为27.5%,儿童为47.1%[1]。肥胖和T2DM密切相关,尤其是内脏肥胖(visceral obesity,VO)易引起胰岛素抵抗(insulin resistance,IR)和高胰岛素血症,进而导致T2DM的发生[2]。VO主要表现为内脏脂肪组织异常聚集,进而出现各种代谢异常,如IR、脂代谢异常、高血压、冠心病等,最终导致不可逆的结局。而内脏脂肪面积(visceral adipose area,VFA)与糖尿病、高血压等疾病的发病率相关[3]。……
