应用解剖型锁定钢板内固定治疗尺骨鹰嘴骨折的病例对照研究

【摘要】 目的：研究解剖型鎖定钢板内固定治疗尺骨鹰嘴骨折患者的效果。方法：选取2017年10月-

2019年6月于本院治疗的尺骨鹰嘴骨折患者76例为观察对象，将患者随机分为观察组和对照组，每组38例。观察组患者采用解剖型锁定钢板内固定治疗，对照组患者采用重建钢板内固定治疗，对两组患者随访1年。比较两组患者的手术时间、开始负重康复训练时间、术中出血量、骨折愈合时间及并发症发生情况，比较两组患者术后1年的肘关节功能情况。结果：观察组的手术时间、骨折愈合时间、开始负重康复训练时间均短于对照组，术中出血量少于对照组，差异均有统计学意义（P<0.05）。观察组患者的并发症发生率为7.89%，与对照组的15.79%比较，差异无统计学意义（P>0.05）。观察组患者术后1年的肘关节功能优良率为84.21%，明显高于对照组的57.89%，差异有统计学意义（P<0.05）。结论：应用解剖型锁定钢板内固定治疗尺骨鹰嘴骨折手术时间短，手术出血量少，术后患者愈合更快，肘关节功能恢复情况更好，值得临床推广。

[Abstract] Objective： To study the effect of anatomical locking plate internal fixation in the treatment of olecranon fractures. Method： A total of 76 patients with olecranon fracture treated in our hospital from October 2017 to June 2019 were selected as the observation subjects， and the patients were randomly divided into observation group and control group， 38 cases in each group. The observation group was treated with anatomical locking plate internal fixation， while the control group was treated with reconstruction plate internal fixation. The patients in both groups were followed up for 1 year. The operative time， time to start weight-bearing rehabilitation training， intraoperative blood loss， fracture healing time and the incidence of complications were compared between the two groups. The elbow function of the two groups was compared 1 year after surgery. Result： The operation time， fracture healing time and start time of weight-bearing rehabilitation training in the observation group were all shorter than those in the control group， and the amount of intraoperative blood loss was less than that in the control group， the differences were statistically significant （P<0.05）. The incidence of complications in the observation group was 7.89%， compared with 15.79% in the control group， and there was no statistical significance （P>0.05）. The excellent and good rate of elbow function in observation group was 84.21%， which was significantly higher than 57.89% of control group， the difference was statistically significant （P<0.05）. Conclusion： The application of anatomical locking plate internal fixation in the treatment of olecranon fracture has the advantages of shorter operation time， less surgical blood loss， faster postoperative healing and better functional recovery of elbow joint， which is worthy of clinical promotion.

尺骨鹰嘴骨折通常为暴力引发的关节内骨折，骨折处常波及半月状关节面，严重影响患者的关节活动，极大地制约着患者的生活便利[1-2]。手术是尺骨鹰嘴骨折常用治疗方法，解剖型锁定钢板内固定、重建钢板内固定等均为其手术方法，不同手术方法的临床应用效果不一[3-5]。因此，寻找合适的尺骨鹰嘴骨折手术方法从而提高其疗效和改善其关节功能十分必要。本文选取2017年10月-2019年6月于本院应用解剖型锁定钢板内固定治疗的尺骨鹰嘴骨折患者与对照组进行比较，目的在于为尺骨鹰嘴骨折寻找合适的固定方式，现报道如下。
